It's a redesigned version of Plague the classic runeword that was originally slated to be released inside Diablo 2 Lord of Destruction v1.15 but was not released.
A few reasons, Blizzard has changed Plague's stats a bit. Plague's order has changed to Cham + Shael + Um. It's which is a significant cost, but also an innovative use of the seldom used Cham rune. The Plague that's been updated Plague seems to be an experimental build that has an extremely specific usage. Everyone's trying to figure out what the advantages of using Plague over something similar to Hoto as well as a potent melee weapon like Greed or the Breath of the Dying.
The main change is that Plague makes use of 3 socketed swords instead of the general weapons. A further change is the -1 bonus at All Skills, and the suppression of +strength because Fal is no more used. A boost in damage has been added, which is a big bonus for raw damage. The other stats are basically identical.
Plague was strange at the beginning, but it did have its uses, possibly for casters; YouTuber Drunski125 says that Plague's usage being restricted to swords alone is a mistake.
